Medicare covers Medical Nutrition Therapy for enrollees who have diabetes and/or Chronic Kidney Disease Stages 3a-5 and not on dialysis.
Medicare covers 3 hours of medical nutrition therapy in the initial year of referral and two hours of medical nutrition therapy for all following years. Additional hours may be available in the same calendar year with a second referral from your doctor when more medical nutrition therapy is needed (we can talk about that!).
Hours are based on a calendar year, so if you don’t use all of your hours by the end of December, then you lose them!
Lauren Gleason is a Medicare Provider, so you may be able to work with Lauren at no out of pocket cost. Please follow these steps to see if you qualify:

Verify your Medicare plan
Lauren is credentialed with Original Medicare. If you have Original Medicare as your primary insurance, you will be eligible to book an appointment at no out of pocket cost and can continue to the next step.
If you have a Medicare Advantage plan or have a primary insurance plan administered by another insurance company, please go back to this page to see if we accept your insurance and proceed with requesting an appointment through that page. If we do not accept your insurance, you may be eligible to enroll in our Three Month Program (more info here) and may utilize HSA or FSA payments to cover the cost. Unfortunately, we are not able to offer individual appointments for Medicare Advantage if we do not accept your insurance carrier. I know, I know. It’s complicated (and frustrating!). If you want to use your insurance benefits to see a dietitian, you will need to find a dietitian that is covered by your insurance.
Obtain a physician referral
Medicare requires a physician referral in order to use your Medical Nutrition Therapy Benefits. Referrals can be faxed to Lauren at 404-868-8136. You MUST obtain a referral prior to booking your appointment. Your referral must include:
- Your name and date of birth
- Your ICD-10 diagnosis code
- Physician full name
- Physician NPI number
